Nate is a talented Administrator with more than 7 years in the Centers Health Care family, and he was our first choice as administrator when Triboro Center first joined the family in 2013. “Running a residential community the size of the Triboro Center is like being the mayor of a small town. And that’s exactly what we are here – a small town with a real neighborhood feel.” Everyone moving into Nate’s community enjoys professional and comprehensive treatment in a warm, friendly atmosphere. His leadership, guidance, and commitment to assembling an exceptional management team is transforming Triboro Center into a very special place.

Nathan started as a financial tracker at Centers corporate, but as he loves helping people in need, he welcomed the opportunity to become an Assistant Administrator. “Whether helping residents during meal time,” he says, “making sure that their needs are met, or simply taking them for walk in their wheelchair, I value my time with our residents.” He also values the dedication and compassion of the Centers team. Nathan is also currently working toward a degree—his goal is to become an Administrator.

Rose has an Associate’s Degree in Secondary Education & Urban Studies from LaGuardia Community College, a Bachelor of Arts in Secondary Education, Hispanic Languages & Literature and Student Personnel & Counseling from CUNY Queens College, and a Certificate of Advanced Study in Therapeutic Recreation from SUNY Cortland. “I look forward to encouraging, exploring and supporting Residents,” she says.

A Social Worker is one of the first friends our new residents make when they arrive at Triboro Center. A graduate of Fordham University, Lorraine knew at a very young age that she wanted to pursue a meaningful career as a professional helper. Shortly thereafter, she found the perfect place to fulfill that dream at Triboro Center. As a qualified social work professional, Lorraine provides support and counseling to our residents and their families, and our world always looks brighter behind her infectious smile.

Director of Admissions

Fabio has been with Centers Health Care since 2010 and has been with Triboro Center for the last 3 years. He has built a solid foundation and is driven by his passion for helping others. As a hard-working family man, Fabio knows placing a loved one can be a difficult process; he is committed to ensuring that the transition is a pleasant experience. His primary goal is making his residents and family members happy with their care and placement, whether it be for short or long term. For the absolute highest quality of care, Triboro Center staffs medical providers on each unit who stay in constant communication with leadership throughout the day. Team collaboration is our forte, and this commitment helps us maintain an unbeatable continuum of care.

Our team also works hard to keep therapies as consistent as possible, tracking all communications through an advanced electronic medical record system. This ensures immediate transmission of information across the entire care network.

We are aware of Governor Cuomo’s recent guidance regarding Nursing Home visitors. We are as excited as you are to welcome you all back to our facility but we must ensure that we do so safely and in accordance with the guidelines set forth.

As of 1/4/2021, in-person visitation is closed. Window visits are offered once a week on Mondays, Tuesdays and Fridays. To schedule a 15-minute window visit, please speak to the Recreation Department at (718) 293-1500 extensions 1011, 1525 or 1526.

Our times to receive care packages are Monday thru Friday from 10am to 11am and 6pm to 7pm only. We reserve the right to refuse care packages outside of these times.
