Nate is a talented Administrator with more than 7 years in the Centers Health Care family, and he was our first choice as administrator when Triboro Center first joined the family in 2013. “Running a residential community the size of the Triboro Center is like being the mayor of a small town. And that’s exactly what we are here – a small town with a real neighborhood feel.” Everyone moving into Nate’s community enjoys professional and comprehensive treatment in a warm, friendly atmosphere. His leadership, guidance, and commitment to assembling an exceptional management team is transforming Triboro Center into a very special place.

Nathan started as a financial tracker at Centers corporate, but as he loves helping people in need, he welcomed the opportunity to become an Assistant Administrator. “Whether helping residents during meal time,” he says, “making sure that their needs are met, or simply taking them for walk in their wheelchair, I value my time with our residents.” He also values the dedication and compassion of the Centers team. Nathan is also currently working toward a degree—his goal is to become an Administrator.

Rose has an Associate’s Degree in Secondary Education & Urban Studies from LaGuardia Community College, a Bachelor of Arts in Secondary Education, Hispanic Languages & Literature and Student Personnel & Counseling from CUNY Queens College, and a Certificate of Advanced Study in Therapeutic Recreation from SUNY Cortland. “I look forward to encouraging, exploring and supporting Residents,” she says.

A Social Worker is one of the first friends our new residents make when they arrive at Triboro Center. A graduate of Fordham University, Lorraine knew at a very young age that she wanted to pursue a meaningful career as a professional helper. Shortly thereafter, she found the perfect place to fulfill that dream at Triboro Center. As a qualified social work professional, Lorraine provides support and counseling to our residents and their families, and our world always looks brighter behind her infectious smile.

Fabio Endara

Fabio has been with Centers Health Care since 2010 and has been with Triboro Center for the last 3 years. He has built a solid foundation and is driven by his passion for helping others. As a hard-working family man, Fabio knows placing a loved one can be a difficult process; he is committed to ensuring that the transition is a pleasant experience. His primary goal is making his residents and family members happy with their care and placement, whether it be for short or long term. Stephanie Minier

Assistant Director

Stephanie graduated from Herbert H. Lehman College with a Bachelor of
Science degree in Therapeutic Recreation. But her education is just the
beginning of what she brings to Triboro Center. Her energy, enthusiasm
and compassion are an inspiration to us all. She is always looking for new
ways to keep our residents engaged and entertained, recognizing the
connection between creative activities and successful rehab. In short, she
brings joy and understanding to everyone in the facility.

For the absolute highest quality of care, Triboro Center staffs medical providers on each unit who stay in constant communication with leadership throughout the day. Team collaboration is our forte, and this commitment helps us maintain an unbeatable continuum of care.

Our team also works hard to keep therapies as consistent as possible, tracking all communications through an advanced electronic medical record system. This ensures immediate transmission of information across the entire care network.

Coronavirus Update

We are excited to share that in compliance with CMS and NYSDOH we are currently open to visitations. You do not need to make a prior appointment in order to see your loved. However please do not visit if you, or someone in your immediate household:

o Are experiencing any symptoms of COVID-19

o Have tested positive for COVID-19 or had close contact with anyone who tested positive for COVID-19 in the past 14 days

o Currently on isolation and/or quarantine based precautions on the recommendations of your local DOH

Please note that visitors will be required to use PPE (personal protection equipment) as well as maintain social distancing during visits at the facility. Additionally, all visitors will have to complete a COVID-19 health screen questionnaire prior to being allowed in to the facility.

Although not required, we encourage all visitors to vaccinate for COVID-19 to enjoy a safe and risk free visit with your loved ones. Please speak with our staff about COVID-19 vaccines. Please note that the current open visitation policy may change based on updated guidance from CMS and/or NYSDOH or based on COVID-19 outbreak protocols, in case of a facility COVID-19 outbreak.
